US Consular Visit to Busan

CONSULAR SERVICES IN BUSAN ON NOVEMBER 13-14 ------------------------------------------------ Officials from the Embassy's American Citizen Services (ACS) Unit will offer consular services in Busan on November 13-14 at the Busan Paradise Hotel. Please note that our hours will be different than in the past. Our services will start at 1:00 p.m. on Monday, November 13. Busan Paradise Hotel, Atlantis Board Room, Floor B1, 1408-5 Joong-Dong, Haeundae-Gu, Busan City Tel. (051) 244-9697. For directions to the Busan Paradise Hotel, please visit: http://www.paradisehotel.co.kr/en/main/ Consular officers will be available on November 13 from 1:00 p.m. ? 4:00 p.m. and on November 14 from 9:00 a.m. ? noon. American citizens will be able to apply for U.S. passports (or additional visa pages in their current passports), Consular Reports of Birth Abroad, and Social Security cards. Consular officers will also offer notarial services, and provide information regarding voting, federal benefits, and registration with the Embassy. Any questions about requirements, particularly for Consular Reports of Birth Abroad, should be directed by email to [email protected]. Please allow at least three days for us to respond. If you plan to visit the ACS Unit in Busan, we strongly urge you to consult the website at http://www.asktheconsul.org for more information on our consular services and to ensure that you bring the proper forms (filled out in advance) and fees with you. In addition to bringing original supporting documents (i.e., birth certificate, Certificate of Naturalization), please bring photocopies of all documents you plan to submit. This will spare you the hassle of trying to find a shop near the hotel to make photocopies and allow us to return the originals to you the same day. Please note that fees must be paid in cash (U.S. dollars or Korean won). Need Additional Pages in Your Passport during the Consular Visit in Busan? -------------------------------------------------------------------------- If you notify us prior to noon on Thursday, November 9, we can add pages to your passport while you wait at the hotel in Busan. Please complete form DS4085. You can download form DS4085 at this address: http://foia.state.gov/FORMS/Passport/DS4085.pdf. Write ?Busan - Extra Pages? at the top of the completed form, include a copy of your passport?s personal data page (the page with your photo), and fax to our office at (02) 397-4101. Alternatively, you can e-mail us your request; be sure to attach clearly scanned copies of both your DS4085 form and your passport?s data page. Send it to [email protected] and put ?RESEND BUSAN EXTRA PAGES? in your subject line. Please note that additional pages will NOT be added to one-year emergency passports or to damaged passports.
